Notes: All new Clubs must on application submit a copy of the Club Constitution including player protection clause. Relevant Fees for ALL clubs is £60.00 league fees + £50.00 Good faith bond (if you paid £50 at the start of the 2010/2011 season to the GADFYL then no payment is necessary as the GFB applies to NEW CLUBS ONLY) + £13.00 Western Regional Development Fee. Cheques/Postal Orders should be made payable to GADYFL and fees must be paid by FRI 15th JULY 2011 or the league fees will rise to £70.00 therafter for clubs paying after the stated date. It is the responsibilty of each league to issue Association Application Forms.

Please print out this form and get it signed by all named parties on form and send it to Andy Sandilands, 49 Iona Ridge, Hamilton ML3 8PZ along with appropriate fees as above, It is also your responsibility to affiliate to the SYFA and send forms to Andy along with a SAE to Hampden and SYFA cheque for him to sign forms and send onto Hampden. This form must also be signed by all named parties on form.
